What Is IgM?

Immunoglobulin M (IgM) is one of the five main classes of antibodies produced by your immune system. Antibodies are specialized proteins made by white blood cells called B lymphocytes (B cells), and they help your body recognize and defend against bacteria, viruses, and other foreign invaders.

IgM is often described as the body’s “first responder” antibody. When your immune system encounters a new pathogen for the first time, IgM is typically the earliest antibody produced. It is a large, star-shaped molecule that is very effective at clumping together germs and activating other parts of the immune defense. Over time, as an infection progresses, your body usually shifts to producing other antibodies such as IgG, which provide longer-lasting protection.

The test may be ordered as part of a broader immunoglobulin panel that also includes IgG and IgA. A clinician may recommend IgM testing to help:

  • Evaluate recurrent or unusual infections
  • Investigate possible immune deficiencies
  • Assess certain autoimmune or inflammatory conditions
  • Help identify a recent or acute infection
  • Monitor certain blood or bone marrow disorders

In some situations, a clinician may order a specific IgM test (for example, IgM antibodies against a particular virus) rather than a total IgM level, to help determine whether a recent infection occurred.

Reference ranges can vary between laboratories depending on the testing method used. Ranges also differ by age, particularly in infants and children whose immune systems are still developing. The values below represent commonly used adult reference ranges.

What High Levels Can Mean

  • Recent or acute infection — since IgM rises early, elevated levels can suggest a new or ongoing infection (viral, bacterial, or parasitic)
  • Chronic infections — some long-standing infections may keep IgM elevated
  • Autoimmune conditions — certain disorders in which the immune system targets the body’s own tissues
  • Liver conditions — some forms of liver disease can be associated with elevated IgM
  • Certain blood disorders — a notably high IgM may prompt a clinician to investigate conditions affecting antibody-producing cells, such as Waldenström macroglobulinemia

Because these possibilities range widely in significance, a high IgM result should always be evaluated by your clinician alongside your symptoms, history, and other lab findings.

What Low Levels Can Mean

A lower-than-expected IgM level may indicate that your body is producing fewer of these antibodies. Low IgM can be associated with:

  • Primary immunodeficiencies — inherited conditions that affect antibody production, sometimes identified in childhood
  • Selective IgM deficiency — a reduced IgM level while other antibody classes remain normal
  • Secondary causes — certain medications, medical treatments, or conditions that suppress immune function
  • Protein loss — conditions in which the body loses proteins, potentially lowering antibody levels

Some people with mildly low IgM have no symptoms at all, while others may experience more frequent infections. Your clinician can help determine whether a low result is meaningful for you.

How the Test Is Done

An IgM test is a standard blood test. A healthcare professional draws a small sample of blood from a vein in your arm, usually a quick and straightforward procedure. The sample is then sent to a laboratory for analysis.

In most cases, no special preparation is required, though your clinician may give you specific instructions if the test is being run alongside others. As with any blood draw, you may experience minor, temporary discomfort or a small bruise at the puncture site.
